Senior Care Centre
[At Woodlands and Kovan]

Every centre serves 40 users per day.

The Community Rehabilitation service provides physiotherapy and occupational therapy for adults and elderly who require clinical follow up rehabilitative care following their discharge from hospitals and clinics.

The Maintenance Day Care and Dementia Day Care admit community clients who require or can benefit from Day Care Services.

Community Rehabilitation

Community Rehabilitation Centre provides sessional physiotherapy and occupational therapy services which include but not limited to:

  • Community Integration activities
  • Functional mobility training and gait training
  • Active and passive exercises to improve or restore range of motion, physical strength, flexibility coordination, balance and endurance
  • Advice on the use of assistive ambulatory devices, orthosis, prosthesis or assistive/adaptive devices to maintain or improve ADL performances
  • Co-ordination and dexterity activities
  • Advice on occupational ergonomics
  • Home assessment and recommendations on home modification
  • Intervention addressing cognition, perception and psychosocial status
  • Caregiver training and client education
  • Wheelchair and Seating assessment
Maintenance Day Care and Dementia Day Care

We practise Person-Centred Care Philosophy and Multidisciplinary Approach to all our Maintenance Day Care and Dementia Day Care clients.  Our services and activities are holistic and minimally cover Custodial Care Services and Maintenance Activities.

 

The Objectives of Maintenance Day Care are :
  1. To provide a supportive centre-based environment to improve or maintain the clients’ quality of life.
  2. To provide Custodial Care services which include, but not limited to :
    • Assistance with Activities of Daily Living,
    • Personal hygiene activities,
    • Assistance with ambulation/ mobility/ transfers,
    • Assistance with oral feeding,
    • Meals and meals preparation, and
    • Assistance with medications.
  1. To provide Maintenance Activities (e.g. physical, cognitive, social-recreational activities) to the clients through Person-Centred Care Philosophy and Multidisciplinary Approach.
  2. To maintain functional ability through meaningful and culturally appropriate activity.
  3. To maintain and/or improve the clients’ general, physical, psychosocial overall well-being.
  4. To Maximize independence and autonomy during activity engagement.

 

The Objectives of Dementia Day Care are :
  • For clients who are diagnosed with Dementia by a Singapore Medical Council-Registered Medical Practitioner.
  • Include all service objectives of Maintenance Day Care.
  • To understand the client’s background (e.g., personality, past and present roles, likes and dislikes).
  • To empower the person with dementia through an enhanced model of Dementia Care.
  • Where applicable, to assess and manage behaviours of concern through non-pharmacological methods, conduct home visits to assess clients, and work with family/caregivers to manage challenging behaviour.
Operating Hours:

Monday to Friday: 8.00 am to 6.00 pm excluding public holidays.

Meals

Morning Coffee / Afternoon Refreshment: Snacks & Beverages
Lunch : Nutritious vegetarian meals.

Fish or Chicken every Mondays and Wednesday except 1st and 15th Day of Month of the Chinese Calendar.

Transport

Adaptive transportation fitted with hydraulic lift and manned by therapy/healthcare assistant is available for Rehab and Daycare clients within Sembawang, Woodlands, Marsiling and Admiralty areas. For the Woodlands Centre, and within Hougang, Serangoon, Kovan, and SenKang for the Kovan Centre.

Getting There

By Public Transport:
The nearest MRT station is Woodlands MRT. Take bus feeder service number 911 from Berth 6 from Woodlands Bus Interchange. Alight at the Woodlands North Plaza Shopping Mall. Man Fut Tong Nursing Home is located at the rear of the Mall.

Driving:
Approach to Nursing Home is from CTE to SLE and exit at Woodlands Ave 2.
