12.2.1 Residential Care

Question: How does one apply for Nursing Home Residential Services?
Answer: Only nonsubsidised full paying residents can apply directly to MFTNH for Residential Care. Those in need of government subsidy are required to approach MSW of the respective hospital to submit an application to Agency for Integrated Care indicating their preference for admission to man fut Tong Nursing Home. Caregiver of full Paying residents can call MFTNH and check on the availability of vacant beds. Application form can be downloaded from www.mft.org.sg or available at the Reception Counter of MFTNH have to be completed and forwarded to the Medical Social Worker / Case Manager along with the latest Medical Report & Chest X-ray of the Applicant. Applications will be assessed and appointment made for an interview, within 3 working days of receipt of Application Form.  If found suitable, 2 working guarantors will be required to sign the Standard Agreement Form

Question: How does one apply for Respite Care Services?
Answer: Same as above non-subsidised Full Paying residents

Question: What is the purpose of providing Respite Care services?
Answer: To give family members a break from daily routines & stress they may experience while providing extra care to their parents. Enable families to go on a holiday / overseas trip, while their elderly who are unable to travel can be cared for, in a safe and comfortable place.

Question: How long can a person stay in MFTNH under the Respite Care Scheme?
Answer: Minimum 3 days to Maximum 30 days.

Question: What is included in the Ward Charges?
Answer:

  • Bed, Meals, Laundry
  • Medical Consultations
  • Nursing Care
  • Physiotherapy
  • Occupational Therapy
  • Medical Social Work Service
  • Dietetic & Pharmacist Services
  • Standard Medical & Surgical Supplies
  • Social & Recreational Activities
  • Admin & Support Services

Question: Will any extra charge be incurred?
Answer: This will be based on individual needs and for the following:

  • Non-Standard Medical & Surgical supplies
  • Special Feeds and Fluids
  • Body holders, restraints, colostomy bags, diapers
  • In-house Specialist consultations - Geriatrician, Psycho-Geriatrician
  • External Laboratory Investigations
  • Bed reservation during hospitalization
  • Death Certificate

Question: What kind of Food is served?
Answer:

  • Nutritious lacto-ovarian vegetarian meals
  • Special diets for residents with specific medical conditions
  • Non vegetarian food is served on 15 & 30 day of every month.
  • Refreshments available 24 hours

12.2.2 Day Rehabilitation and Day Care Centre (Manchu Please go through)

Question: How does one apply to enroll in Day Rehabilitation Centre / Day Care  centre?
Answer: Application form can be downloaded from www.mft.org.sg or available at the Reception Counter of MFTNH. The form has to be completed and forwarded to the Manager, DRC/DCC along with the latest Medical Report of the applicant. Applications will be assessed and appointments made for an interview, within three working days of receipt of application form. If the applicant is found suitable, two working guarantors are required to sign the Standard Agreement Form.

Question: What kind of services do you provide?
Answer: We provide Physiotherapy and Occupational Therapy services. Applicants can choose to have an hourly therapy program, a full day therapy and day care program or only day care program.

Question: What is the Scheme of Charges for DRC/DCC?
Answer: Scheme of charges for Day Rehabilitation & Day Care Services varies between $100/- to $600/- per month or $6/- to $60/- per day depending on type of services and level of subsidy. To view scheme of charges for DRC/DCC please click here.

Question: Do you provide subsidy?
Answer: Yes, subsidy is available up to 75% based on MOH Means Test Frame work.

Question: Do you provide Transport Services for DRC/DCC Clients?
Answer: Yes. Adaptive transportation fitted with hydraulic lift and manned by therapy / healthcare assistant is available for DRC/DCC clients within Sembawang, Woodlands, Marsiling and Admiralty areas.

Question: Do you charge separately for Transportation Services?
Answer: For those opting for monthly package, transportation charges are included and for those opting for daily charges, a transportation fee of $3/- to $12 is applicable for two way transportation depending on subsidy level.

Question: Do you have Stair Crawl?
Answer: Yes, we have stair crawl for those who are non ambulant and staying at non lift landing HDB apartments.

Question: Do you accept PA Cases?
Answer: Yes, we accept PA cases.

Question: What is your DRC/DCC operating hours?
Answer: DRC/DCC operates Monday to Friday from 8.00 am to 6.00 pm excluding public holidays.

Question: What kind of Food is served?
